Одним из основных преимуществ КТК по сравнению с эндоскопической колоноскопией является более простая подготовка толстой кишки благодаря уменьшению объема слабительных препаратов. Тем не менее очищение толстой кишки, включающее в себя в том числе соблюдение диеты, является наиболее обременительным аспектом всей процедуры. На настоящий момент не существует единого мнения о преимуществе какого-либо варианта диеты для подготовки толстой кишки к КТК, и данный вопрос требует дальнейшего изучения.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ: Сравнение эффективности и переносимости однодневной и трехдневной диеты при подготовке к КТ-колонографии.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ: В исследование было включено 107 пациентов. 55 пациентов придерживались трехдневной диеты (3ДД), 52 — однодневной диеты (1ДД). Проводилась оценка каждого из шести сегментов толстой кишки на предмет остаточного объема жидкости и стула по 4-балльной шкале (4 — отсутствие; 3–1 — увеличение количества жидкости или стула). Качество маркировки остаточного содержимого оценивали по значению плотности в единицах Хаунсфилда и однородности (значения стандартного отклонения в регионе интереса). Для оценки тяжести соблюдения диеты пациентов просили отметить, если соблюдение диеты они сочли сложным. Также оценивалась готовность пациентов к повторению процедуры в будущем.</p></sec><sec><title>Статистика</title><p>Статистика: Сравнение групп после проверки на нормальность распределения осуществлялось с помощью U-критерия Манна-Уитни для количественных переменных и с помощью критерия χ2 Пирсона для категориальных данных. Значение p&lt;0,05 считалось статистически значимым. Для оценки согласия между исследователями был проведен каппа-анализ Коэна. Все расчеты проводились с использованием программного обеспечения для статистической обработки Jamovi версия 1.2.27.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ: Средний балл остаточной жидкости составил 37 (Q1–Q3 35–39) для 3ДД и 36 (Q1–Q3 34–38) для 1ДД, остаточного стула — 48 (Q1–Q3 46–48) и 48 (Q1–Q3 47–48) соответственно. Медиана плотности остаточной жидкости составила 887 HU (Q1–Q3 725–1238 HU) и 953 HU (Q1–Q3 668–1203 HU), медиана значений однородности — 77 HU (Q1–Q3 55– 86 HU) и 59 HU (Q1–Q3 47–92 HU) соответственно. Достоверных различий по этим параметрам между группами не было (p&gt;0,05). Соблюдение диеты было статистически значимо легче в группе 1ДД по сравнению с 3ДД (p=0,012): 26 пациентов (47,3%) из группы 3ДД и 13 пациентов (25%) из группы 1ДД отметили сложность соблюдения диеты. Готовность к повторению процедуры была достоверно выше в группе 1ДД 75,0% (39 пациентов), чем в группе 3ДД,— 54,5% (30 пациентов) (р=0,027).</p></sec><sec><title>ОБСУЖДЕНИЕ</title><p>ОБСУЖДЕНИЕ: Однодневная диета перед КТК дает оптимальное качество подготовки, а изменение диеты с трехдневной на однодневную повышает переносимость подготовки в целом, а также увеличивает готовность пациентов повторить процедуру в будущем. Также исследование показало отсутствие преимуществ у диеты с отказом от приема твердой пищи при подготовке к КТК. При обоих вариантах диеты (однодневная и трехдневная) качество подготовки было высоким, а статистически значимые различия в качестве подготовки между группами отсутствовали.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ: Изменение диеты с трехдневной на однодневную не влияет на качество подготовки к КТК, но повышает переносимость процедуры пациентами, а также их готовность повторить процедуру.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>INTRODUCTION</title><p>INTRODUCTION: CT colonography (CTC) is a highly accurate, minimally invasive method of intraluminal imaging of the colon using computed tomography. One of the main advantages of CTC compared to endoscopic colonoscopy is easier colon preparation due to the reduced volume of laxatives. However, colon cleansing, which includes dieting, is the most burdensome aspect of the entire procedure. At present, there is no consensus on the benefits of any dietary option for preparing the colon for CTC, and this issue requires further study.</p></sec><sec><title>OBJECTIVE</title><p>OBJECTIVE: Comparison of the efficacy and tolerability of a one-day and a three-day diet at CT colonography (CTC).</p></sec><sec><title>MATERIAL AND METHODS</title><p>MATERIAL AND METHODS: 107 patients were enrolled in this study. 55 patients adhered to a three-day diet (3DD) and 52 to a one-day diet (1DD). Each of six colonic segments was scored for residual fluid and stool using a 4-point scale (4 — absence; 3–1 — increasing fluid or stool). The quality of fecal tagging was assessed with fluid attenuation (HU values) and homogeneity (SD values). In addition, subjective severity of dieting and readiness to repeat the procedure were assessed.</p></sec><sec><title>Statistics</title><p>Statistics: Comparison of groups after testing for normality of distribution was carried out using the Mann-Whitney U test for quantitative variables and using the Pearson χ2 test for categorical data. A p value &lt;0.05 was considered statistically significant. Cohen’s kappa analysis was performed to assess interobserver agreement. All calculations were performed using statistical software Jamovi version 1.2.27.</p></sec><sec><title>RESULTS</title><p>RESULTS: The mean residual fluid score was 37 (Q1–Q3 35–39) for 3DD and 36 (Q1–Q3 34–38) for 1DD, residual stool — 48 (Q1–Q3 46–48) and 48 (Q1–Q3 47–48) respectively. The median residual fluid attenuation was 887 HU (Q1–Q3 725– 1238 HU) and 953 HU (Q1–Q3 668–1203 HU), the median of homogeneity — 77 HU (Q1–Q3 55–86 HU) and 59 HU (Q1– Q3 47–92 HU). There was no significant difference in these parameters between groups (p&gt;0.05). Difficulty of dieting was statistically significantly higher in 3DD group compared to the 1DD group (p=0.012): 26 patients (47.3%) from the 3DD group and 13 patients (25%). Readiness to repeat the procedure was significantly higher in 1DD group 75,0% (39 patients) than in 3DD group 54,5% (30 patients) (p=0.027).</p></sec><sec><title>DISCUSSION</title><p>DISCUSSION: A one-day diet before CTC provides optimal quality of preparation, and changing the diet from a three-day to a one-day increases the tolerability of the preparation in general, and also increases the willingness of patients to repeat the procedure in the future. The study also showed that there was no benefit to a clear liquid diet in preparation for CTC.
