Лучевая терапия со снижением дозы на область критических сосудистых эректильных структур входит в число таких методов.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ: Определение оптимальных параметров дозиметрического планирования сосудосберегающей стереотаксической лучевой терапии (СС-СТЛТ) РПЖ.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ: В исследование вошли 20 пациентов с верифицированным РПЖ групп низкого и промежуточного риска (NCCN 2023). Лучевое лечение было представлено стереотаксической лучевой терапией (СТЛТ) в виде пяти фракций облучения по 7,25 Гр. Помимо стандартных критических органов, идентифицировались критические сосудистые структуры, связанные с  эректильной функцией (эректильные критические органы  — ЭрКО): луковица полового члена (ЛПЧ), ножки кавернозных тел (НКТ) и внутренние половые артерии (ВПА). Для каждого пациента рассчитывалось четыре дозиметрических плана (ДП): сосудосберегающий (ДП-CC); стандартный (ДП-C); сосудосберегающий план, рассчитанный при формировании общепринятого анизотропического отступа в 5 мм для планируемого объема облучения и отступом в сторону прямой кишки — 3 мм (ДП-5/3); сосудосберегающий план для облучения с использованием 0,5-сантиметрового многолепесткового коллиматора (ДП-0,5). Все планы, за исключением ДП-0,5, были сгенерированы для линейного ускорителя с 0,25-сантиметровым многолепестковым коллиматором.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ: Во всех дозиметрических планах удалось достигнуть требуемых значений показателей, характеризующих качество покрытия мишени, равно как и  уложиться в  предписываемые лимиты по  основным критическим органам. Оптимальное распределение с точки зрения снижения дозы на ЭрКО достигнуто в ДП-СС. ДП-СС в сравнении с ДП-С позволил существенно снизить лучевую нагрузку на  область НКТ и  ВПА  — D2% уменьшилось на  30% (р0,05).</p></sec><sec><title>ОБСУЖДЕНИЕ</title><p>ОБСУЖДЕНИЕ: Полученные результаты свидетельствуют, что СС-СТЛТ реализуема у подавляющего числа пациентов, страдающих РПЖ. Значимое снижение подводимой дозы на область ЭрКО может быть достигнуто на большинстве линейных ускорителей электронов, оснащенных современными многолепестковыми коллиматороми.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ: B Наибольший вклад в снижение лучевой нагрузки на ЭрКО при СС-СТЛТ РПЖ вносит сокращение стандартных отступов при формировании PTV, в то время как ширина лепестков коллиматора не оказывает существенного влияния на величину подводимой к этой области дозы.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>INTRODUCTION</title><p>INTRODUCTION: Тhe development of new methods of radical treatment of prostate cancer (PCa) aimed at maintaining a high quality of life is the most important task of modern oncourology. Radiation therapy with a dose reduction to the area of critical vascular erectile structures is one of them.</p></sec><sec><title>OBJECTIVE</title><p>OBJECTIVE: Сompare different approaches for «vessel-sparing» stereotactic radiation therapy (VS-SRT) PCa.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: Тhe study included 20 patients with histologically confirmed PCa of low and intermediate risk of recurrence (NCCN). SRT was planed at five fractions of 7.25 Gy. During planning we ad to the standard critical organs, vascular structures that are responsible for erectile function (ErOAR)): penile bulb (PB), crura of the cavernous bodies (CCB) and internal pudendal arteries (IPA)). For each patient, four dosimetric plans (DP) were calculated: «vessel-sparing» (DP-VS); standard (DP-S); «vessel-sparing» plan calculated in the formation of the generally accepted anisotropic indent of 5 mm for the planned volume of irradiation and a step towards the rectum — 3 mm (DP-5/3); «vessel-sparing» plan for irradiation using a 0.5-centimeter multi-life collimator (DP-0.5). All dosimetry plans, with the exception of DP-0.5, were generated for a linear accelerator with a 0.25 cm multi-leaf collimator.</p></sec><sec><title>RESULTS</title><p>RESULTS: Аll dosimetry plans, were cherecterized by exellent target coverage abd meet the prescribed limits for the critical organs. Most efficient sparing of ErOAR was achieved in DP-VS. The advantages of DP-VS in comparison with DP-S was significant reduction (30%) of D2% for CCB (IPA) (р0.05).</p></sec><sec><title>DISCUSSION</title><p>DISCUSSION: Тhe obtained results indicate that VS-SRT is feasible in the vast majority of patients suffering from PCa.
