Для дифференциальной диагностики выполнена магнитно-резонансная томография (МРТ) малого таза с внутривенным контрастированием препаратом гадобутрол (Гадовист) и DWI.</p><p>МРТ выявила образование, соответствующее эндометриоме, с внутрипросветным солидным узлом (максимальный размер ≈22 мм), демонстрирующим умеренное неоднородное контрастное усиление. Критически важным признаком доброкачественности процесса явилось отсутствие значимого ограничения диффузии: измеряемый коэффициент диффузии (ADC) узла составил 1,5–1,8×10–3 мм2/с. Признаков инвазивного роста, лимфаденопатии или канцероматоза не выявлено. </p><p>ЗАКЛЮЧЕНИЕ. Комплексная мультипараметрическая МРТ, включающая анализ морфологии образования, кинетики контрастного усиления и, особенно, количественных показателей ADC, является высокоинформативным методом для дифференциальной диагностики децидуализированной эндометриомы и злокачественной опухоли яичника у беременных, что позволяет обосновать выбор консервативной тактики ведения и избежать необоснованного оперативного вмешательства.</p></abstract><trans-abstract xml:lang="en"><p>Decidualized endometriomas in pregnancy are a rare finding and pose a significant diagnostic challenge as they mimic malignant ovarian neoplasms. We describe a clinical case of a 27-year-old patient at 15 weeks of gestation with a solid-cystic mass of the left ovary detected on ultrasound. MRI of the pelvis with intravenous contrast enhancement with gadobutrol (Gadovist) and DWI was performed for differential diagnosis. MRI revealed a mass consistent with an endometrioma with an intraluminal solid nodule (height &lt;11 mm) that showed moderate heterogeneous contrast enhancement. A critically important feature was the absence of significant diffusion restriction: the ADC value of the node was 1.5–1.8×10–3 mm2/s. There were no signs of invasive growth, lymphadenopathy, or carcinomatosis.</p><p>CONCLUSION: Comprehensive MRI evaluation, including analysis of morphology (nodule height), contrast enhancement kinetics, and especially quantitative ADC values, is a highly informative method for the differential diagnosis of decidualized endometrioma and malignant ovarian tumors in pregnant patients, justifying the choice of conservative tactics and avoiding unjustified surgical intervention.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>децидуализированная эндометриома</kwd><kwd>беременность</kwd><kwd>магнитно-резонансная томография</kwd><kwd>диффузионно-взвешенная визуализация</kwd><kwd>ADC</kwd><kwd>дифференциальная диагностика</kwd></kwd-group><kwd-group xml:lang="en"><kwd>decidualized endometrioma</kwd><kwd>pregnancy</kwd><kwd>magnetic resonance imaging</kwd><kwd>diffusion-weighted imaging</kwd><kwd>ADC</kwd><kwd>differential diagnosis</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Rozalli F.I., Rahmat K., Fadzli F. et al.
