В статье представлены особенности течения (клинические симптомы, методы диагностики, гистологические особенности, прогноз и ответ на терапию) различных поражений легких при системной красной волчанке. К первичным поражениям, ассоциированным непосредственно с основным заболеванием, относятся плеврит, острый люпус-пневмонит, интерстициальные пневмонии, легочные (альвеолярные) геморрагии, легочная гипертензия, легочный васкулит и тромбоэмболии легочных артерий. Синдром «сморщивающегося легкого», осложнения сердечной и почечной недостаточности, инфекционные пневмонии и токсические (лекарственные) поражения относятся к вторичным поражениям. Все клинические примеры дополнены собственными рентгенологическими наблюдениями.

The article reviews the clinical symptoms, imaging techniques, histopathology, prognosis, and response to treatment of pulmonary manifestations of systemic lupus erythematosus. Manifestations directly associated to the disease include pleuritis, acute lupus pneumonitis, interstitial lung disease, pulmonary (alveolar) hemorrhage, pulmonary hypertension, pulmonary vasculitis and pulmonary thromboembolic disease. Shrinking lung syndrome, complications due to heart and renal failure, opportunistic pneumonia and drug toxicity are secondary manifestations. All clinical cases supplement own radiological observations.
