В исследовании выполнена оценка нейроваскуляризации с помощью контрастной и бесконтрастной магнитнорезонансной перфузии. Материалы и методы. В исследование включили 140 пациентов с СД 2-го типа с когнитивными нарушениями и без таковых. Проводили оценку вариабельности гликемии, магнитно-резонансную томографию (МРТ) — контрастную и бесконтрастную. Результаты. Изменение нейроваскуляризации головного мозга ассоциировано с вариабельностью гликемии, при этом преимущество выявлено при проведении контрастного исследования. Нарушение микроциркуляции происходит как в корковых (в области серого вещества), так и в подкорковых структурах. Наибольшее влияние на нарушение васкуляризации головного мозга при СД 2-го типа выявлено в отношении возраста, индекса массы тела, артериальной гипертензии. Гипергликемия и ее длительность, а также гипогликемия, качество контроля, средняя амплитуда колебания, среднее время нахождения в целевом диапазоне определяли изменение васкуляризации. Выводы. Понимание патогенеза нарушения микроциркуляции и влияния вариабельности гликемии на головной мозг при СД 2-го типа дает возможность своевременно корректировать гликемическую кривую и устранить модифицируемые факторы риска (индекс массы тела, артериальной гипертензии), влияющих на развитие когнитивных нарушений.</p></abstract><trans-abstract xml:lang="en"><p>Objective. To evaluate neurovascularization using contrast and non-contrast magnetic resonance perfusion. Materials and methods. 140 patients with type 2 diabetes with and without cognitive impairment were included in the study. We assessed the variability of glycemia, magnetic resonance imaging (MRI): contrast and non-contrast. Results: brain neurovascularization change is associated with glycemia variability, with contrast advantage revealed in the contrast study. Microcirculation disorder occurs in both cortical (grey matter area) and subcortical structures. The greatest influence on brain vascularization disturbance in type 2 diabetes was revealed in terms of age, body mass index, arterial hypertension. Hyperglycemia and its duration, as well as hypoglycemia, control quality, average amplitude of oscillation, average time in the target range determined the change of vascularization. Conclusions.
