Несмотря на множество доступных лучевых методов обследования, на сегодняшний день, нередко возникают трудности в  диагностике очаговых образований почек. Компьютерная томография (КТ), на  сегодняшний день, является «золотым стандартом» лучевой диагностики рака почки (РП), которая за годы своего существования доказала высокую эффективность. Ультразвуковое исследование (УЗИ) является оптимальным методом первичной диагностики солидных образований почек. Мультипараметрическое УЗИ (мпУЗИ) с применением эхоконтрастирования значительно дополняет полученную в рамках традиционного сканирования информацию, существенно расширяя спектр решаемых задач. В статье рассматриваются современные возможности лучевых методов в выявлении и оценке образований почек, их роль в определении тактики ведения пациентов.Цель исследования: изучить возможности и сравнить эффективность отдельных методик мультипараметрического УЗИ (Врежим, ЦДК и ЭДК, эхоконтрастирование) в оценке солидных образований почек между собой и компьютерной томографией (КТ), как референтным методом лучевой диагностики.Материалы и методы. Мультипараметрическое ультразвуковое исследование (мпУЗИ) с применением В-режима, допплеровского картирования и контрастного усиления (1,0 мл Соновью) проведено 79 пациентам с солидными образованиями почек. Большая часть выявленных образований (n=65) гистологически верифицированы, остальные (n=14), имеющие признаки доброкачественных изменений, подвергались динамическому контролю. В  качестве контрольного метода всем пациентам выполнялась компьютерная томография с контрастированием.Результаты. Эффективность В-режима составила: чувствительность (Ч) — 76,9%; специфичность (С) — 29,6%; точность (Т) — 60,7%, для режима ЦДК данные показатели составили Ч — 80,7%; С — 37,1%; Т — 65,8%. Применение контрастного усиления значимо повысило общую эффективность мультипараметрического УЗИ: Ч — 92,1%; С — 93,7%; Т — 92,4% и показало сопоставимую эффективность с КТ: Ч — 94,9%; С — 85,0%; Т — 92,4%.Заключение. КТ по сей день остается «золотым стандартом» в оценке образований почек, однако мпУЗИ с контрастным усилением за счет сопоставимой эффективности, а также с учетом отсутствия лучевой нагрузки, широкой доступности оборудования, высокой разрешающей способности, при наличии противопоказаний к  КТ в  ряде случаев может выступить в качестве метода выбора</p></abstract><trans-abstract xml:lang="en"><p>Introduction. Today, we have a lot of difficulties in the focal kidney lesions diagnosis, despite the many available radiation methods of examination. Computed tomography (CT) is the «gold standard» of radiological diagnosis of kidney cancer (KC), which over the years of its existence has proven to be highly effectiveness. Ultrasound examination (US) is the best method for the primary diagnosis of solid kidney lesions. Multiparameter ultrasound (mpUS) with using contrast significantly supplements the information obtained in traditional scanning, significantly expanding the range of tasks to be solved. The article discusses the modern possibilities of radiology methods in the detection and assessment of kidney lesions, their role in determining the patient management tactics.Objective. Тo explore the possibilities and compare the effectiveness of individual multiparameter ultrasound techniques (Bmode, CDI and PDI, ultrasoundcontrast) in assessing solid kidney formations with each other and computed tomography (CT), as a reference method of radiation diagnostics.Material and methods. Мultiparametric ultrasound examination (mpUS) using B-mode, doppler mapping and contrast enhancement (1,0 ml Sonovue) was performed in 79 patients with solid kidney lesions. Most of the detected lesions (n=65) were histologically verified, the rest (n=14) with signs of benign changes were subjected to dynamic control. All patients underwent contrast-enhanced computed tomography as a control method.Results. Тhe effectiveness of the B-mode was: sensitivity (Se) — 76,9%; specificity (Sp) — 29,6%; accuracy (A) — 60,7%, for the CDI mode, these indicators were Se — 80,7%; Sp — 37,1%; A — 65,8%. The use of contrast enhancement significantly increased the overall efficiency of multiparameter ultrasound: Se — 92,1%; Sp — 93,7%; A — 92,4% and showed comparable efficiency with CT: Sp — 94,9%; Se — 85,0%; A — 92,4%.Conclusions.
